For a successful transition to The Dean ongoing care for the
resident and family is managed by a multidisciplinary team that
meets together regularly.
The team, according to resident needs, can include:
Consultant in Rehabilitation- Occupational therapist
- Physiotherapist
- SALT
- Dietician
- Psychology
- Counselling
