Person centred care involves tailoring a person’s care to their interests, abilities, history and personality. At The Yorkshire Clinic we put the person at the heart of everything we do.
This helps to ensure that people living with Dementia can continue to take part in the things they enjoy which can be an effective way of preventing and managing the symptoms of Dementia.
Family, carers and the person with Dementia will always be involved in developing a care plan based upon person centred care. Their knowledge and understanding of the person is extremely valuable to ensure that the care plan is appropriate.
We strive to involve carers of people with Dementia from the moment they start their journey in our hospital until the moment of discharge working in partnership with the individual and their families/carers. For those patients who have carers/family who would like to stay – we extend the warmest of welcomes.
The Yorkshire Clinic has committed to John’s campaign:
Please support John's Campaign: for the right to stay with dementia patients in hospital.
