Microsurgical Varicocelectomy/ Varicocele Surgery at North Downs Hospital

Microsurgical varicocelectomy is a precise, minimally invasive procedure that treats varicoceles by tying or clipping abnormal testicular veins using advanced microscopic techniques to improve outcomes and reduce complications.

What are varicoceles?

Varicoceles are abnormally enlarged veins within your scrotum, which can lead to male infertility, persistent pain, or reduced testosterone levels.

Varicocele is recognised as the most frequent cause of infertility in men.¹ Affecting nearly 15% of the male population, this condition can disrupt normal testicular function, impair testosterone production, and significantly reduce fertility potential.2

Timely diagnosis and effective treatment, such as microsurgical varicocelectomy, are essential in managing varicocele and improving both reproductive and overall health outcomes.

What is Microsurgical Varicocelectomy?

Microsurgical varicocelectomy is a highly specialised procedure performed under a high-powered operating microscope. This technique allows for precise identification and isolation of the affected veins, ensuring the highest success rates with minimal risks.

During the surgery, the abnormal testicular veins are tied or clipped through a small incision in the groin, effectively reducing symptoms and improving fertility outcomes.

Microsurgical approaches are often preferred due to their accuracy, minimal invasiveness, and lower complication rates compared to other surgical options. Studies have highlighted its effectiveness in restoring normal testicular function and improving semen parameters, making it a gold-standard treatment for varicoceles.1

What happens on the day of your surgery?

On the day of your procedure, your urologist or a team member will review your medical history and current medications. They will also discuss the surgery with you to ensure all your questions are answered and your consent is confirmed.

Pre-surgery preparations include:

  • Ensuring you’ve fasted as per instructions.

  • Reviewing any pre-operative imaging, such as ultrasound scans.

  • Administering antibiotics to minimize the risk of infection.

Once you’re prepared, you’ll be taken to the operating room, where the procedure will be conducted under general anaesthesia.

How is the procedure conducted?

Microsurgical varicocelectomy typically takes about 30 minutes per side, with a bilateral procedure lasting approximately one hour. After administering anaesthesia, the surgeon makes a small incision in your groin area. Using a high-powered operating microscope, they identify the swollen veins responsible for your varicocele.

The surgeon carefully ties or clips these veins while preserving important structures like arteries and lymphatic vessels, which minimises the risk of complications such as testicular damage or hydrocele formation. The incision is closed with absorbable sutures, which naturally dissolve within two to three weeks.

This meticulous approach significantly improves the success rate of the procedure, offering patients better outcomes with reduced risks.

What can I expect following the procedure?

After the surgery, it’s normal to experience some of the following:

  • Scrotal swelling - that may persist for a few days and is part of the natural healing process.

  • Clear fluid discharge - mild seepage from the wound is common and should subside within a few days.

  • Unchanged external appearance - the varicocele’s outward appearance may not immediately change, but symptoms often improve shortly after the procedure.

  • Temporary discomfort or pain - may last for a few days to a couple of weeks but can be managed with prescribed pain relief.

Your healthcare provider will provide detailed post-operative care instructions to ensure a smooth recovery.

What is the recovery process of Microsurgical Varicocelectomy?

Recovery after microsurgical varicocelectomy is generally quick and straightforward. Most men return to work within 24 to 72 hours, although full recovery to normal activities usually takes around 2 to 3 weeks.

Here’s a general timeline of what to expect during recovery:

  • First few days – rest and limit physical activity. Manage pain with prescribed medication.

  • 1-2 weeks - gradual return to light activities, avoiding strenuous tasks or heavy lifting.

  • 3 weeks onward - resume normal routines, including exercise, based on your doctor’s advice.

Factors such as age, overall health, and adherence to post-operative instructions can influence recovery speed.
